Please bring your child’s insurance card with you to each and every visit.
As part of our contract with insurance companies requiring co-pays, we are required to collect your co-pays at the time of your visit. If you have a high deductible plan we will collect $50 at each visit and bill you for the remaining balance after we receive an Explanation of Benefits from your insurance company.
Our office accepts cash, checks, Visa, MasterCard, Discover and American Express. You can also go online and pay.
